As an Integrated Care Coordinator, your responsibilities will include:

  • Coordinating the care of individuals across mental health, medical, educational, and related domains.
  • Assessing needs through established tools to create an initial care plan.
  • Connecting families with essential resources in their community, including educational, financial, mental health, physical health, housing, and food assistance.
  • Monitoring and reviewing service provision to ensure individuals are actively participating in their care.
  • Maintaining regular contact with all individuals involved in your assigned caseload, in accordance with state regulations.
  • Ensuring timely and accurate completion of necessary documentation, including service records, outcome reports, service plans, and assessments.

The role primarily involves community engagement and home visits to clients, with vehicles provided for work travel.

Qualifications:

  • Education: An Associate's degree with 12 semester credit hours in sociology, social welfare, psychology, gerontology, or a related social science, along with 2 years of experience in public or private human services, including 1 year of direct client interaction. Alternatively, a Bachelor's Degree in a Human Services field (Psychology, Social Work, Sociology, Criminal Justice, etc.) is acceptable.
  • Experience: A minimum of 2 years in public or private human services, with at least 1 year of direct client contact.

Devereux maintains a zero-tolerance policy for abuse and is committed to creating a culture of reporting and cooperation with authorities to mitigate risks.
