Job Summary

Key Responsibilities
  • Evaluate patients referred to home health care services and assess their needs for care.
  • Conduct thorough interviews with patients and their families to determine the appropriateness of services.
  • Obtain necessary documentation from physicians and insurance providers.
  • Complete assessments and transmit results, ensuring all paperwork is submitted in a timely manner.
  • Adhere to agency administrative policies and procedures.
  • Participate in meetings and training sessions as required.
  • Ensure all consents and original intake assessment documents are submitted within 5 days.
Requirements
  • Graduate of a 2-4 year accredited nursing program.
  • Bachelor's Degree in Nursing or advanced degree preferred.
  • Active and unrestricted NY State Registered Nurse (RN) License.
  • Knowledge of current community health practices, including behavioral health, cognitively impaired seniors, and disabled individuals.
Preferred Qualifications
  • Minimum of three to five years of clinical and supervisory experience.
  • Strong assessment and documentation skills.
  • Excellent verbal and written communication skills.
  • High level of professionalism and ability to handle sensitive information.
  • Meticulous attention to detail and strong organizational skills.
  • Proficiency in computerized systems, including Microsoft Outlook, Word, and Excel.
