We are seeking a skilled Licensed Practical Nurse to provide high-quality care to our patients in their homes. This is a full-time position with flexible scheduling options, including 8-hour days and evening shifts.

As a Licensed Practical Nurse with LHC Group, you will have the opportunity to work independently and make a meaningful difference in the lives of our patients. You will be responsible for providing direct nursing care as prescribed by physicians, recording clinical and progress notes, and coordinating other patient care activities.

**Job Summary:**

The Home Care Licensed Practical Nurse is responsible for providing direct nursing care to acutely ill and chronically ill patients in their homes. This role works under the direct supervision of the Registered Nurse and/or designated supervisor.

**Key Responsibilities:

  • Provides high-quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing.
  • Documents patient visits thoroughly and completely per regulatory and payer requirements.
  • Actively participates in the agency's care coordination process, including timely reporting of patient needs and changes in condition.

**Requirements:**

To be successful in this role, you must have current LPN licensure in the state of practice, a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Previous home care experience is preferred, but not required.

**Salary:** The estimated salary for this position is $65,000 - $85,000 per year, depending on experience and qualifications.
