**Estimated Salary:** $48.40/hour (weekdays) to $51.33/hour (weekends)

Job Description:

Care for patients in their homes with our flexible home health program. Perform comprehensive patient assessments, develop individualized care plans, maintain continuity of patient care, and educate patients and caregivers on aftercare and ongoing care.

Required Skills and Qualifications
  • Current RN license
  • 1+ year(s) of Certified Home Health experience as an RN
  • OASIS experience
  • Strong assessment, critical thinking, and communication skills
  • Current CPR certification
  • Comfortability using technical tools and software/EMR for visit documentation
  • Reliable transportation, current driver's license, and car insurance coverage
  • Proof of Immunization/Exemption for Flu and COVID-19
  • TB Test or Chest X-Ray
