We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Holladay Park Plaza, a 5-Star Continuing Care Retirement Community (CCRC) in Portland, OR.
Job SummaryThe LPN will provide high-quality patient-centered care to our residents in our 51-bed Skilled Nursing Facility. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with the interdisciplinary team to ensure seamless care transitions and coordination
- Participate in quality improvement initiatives to enhance resident outcomes and satisfaction
