Job Summary

We invite highly skilled and compassionate registered nurses (RNs) to join our team as Home Health RN Case Managers. As a key member of our care team, you will provide patient-centered care, promote health and independence, and make a meaningful impact on the lives of patients and their families.

Responsibilities
  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
  • Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
  • Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
  • Regularly communicate patient progress to the clinical manager and care team.
  • Plans and provides staff education.
