About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Queen of the Valley Medical Center. As a Clinical Pharmacist, you will play a critical role in providing high-quality, individualized pharmaceutical care to our patients.

Key Responsibilities
  • Develop and implement medication therapy plans to optimize patient outcomes
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication
  • Provide education and support to patients and families on medication use and management
  • Participate in quality improvement initiatives to enhance patient care and safety
Requirements
  • PharmD or Bachelor's Degree in Pharmacy
  • California Pharmacist Registration
  • Basic Life Support-Provider certification (where applicable)
  • Board of Pharmacy Preceptor license (where applicable)
Preferred Qualifications
  • Acute care hospital pharmacy experience
  • Completion of ASHP-accredited PGY1 residency
