Position Overview:
The Licensed Practical Nurse is responsible for executing nursing tasks and providing patient care as directed by the healthcare team, including the Director of Nurses and/or Physician.

Key Responsibilities:
  • Deliver nursing care aligned with individual treatment plans; administer prescribed medications; monitor and report on patient responses to treatments and health status changes; assist with daily living activities and hygiene; and provide health education to patients.
  • Measure and document vital signs such as temperature, blood pressure, and pulse rates using standard procedures.
  • Under the guidance of a Registered Nurse, participate in resident treatment, administer medications, observe and report on changes in resident conditions, and collect specimens for laboratory analysis.
  • Perform basic dressing applications; assist with bathing and feeding residents; maintain cleanliness and order in resident quarters by making beds, changing linens, and assisting with mobility.
  • Provide professional nursing services through assessment, planning, implementation, and evaluation of care.
  • Ensure adequate supplies are available to meet resident needs.
  • Communicate patient requirements to the medical team for coordinated care.
  • Participate in resident admission processes, which may require travel.
  • Contribute to the development and enforcement of policies and procedures.
  • Engage in fiscal management, performance evaluations, infection control standards, wound care, and continuous quality improvement initiatives.
  • May supervise subordinate staff in their resident care duties and assist in facility oversight.
Qualifications:
  • Must hold a current Licensed Practical Nurse license in New York State.
  • At least one year of relevant experience is required.
  • BLS certification is mandatory.
Additional Information:
Shift availability includes primarily evenings and nights, with alternating weekends and some holidays.
Full-Time position with a contract length of up to 2 years.
