About the Role

We are seeking a dedicated Physical Therapist to join our team at Elite Home Health, a part of LHC Group. This is an excellent opportunity for a skilled therapist to provide high-quality patient care in a home setting.

Job Summary

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, they will work with the patient and healthcare team to determine a treatment plan and implement interventions aimed at improving and enhancing the patient's well-being.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, assisting physicians in developing personalized therapy plans.
  • Observe and report patients' responses to treatment and any changes in their condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards of practice.
  • Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification.
  • A valid driver's license and vehicle insurance, as well as access to a dependable vehicle.
Compensation and Benefits

We offer a competitive salary range of $83,000 - $110,000 per year, commensurate with experience, as well as a comprehensive benefits package, including holiday pay, medical, dental, vision benefits, 401(k) retirement plan, continuing education opportunities, and employee assistance programs.
