Qualifications:
Licensed Rehabilitative Specialist
This position welcomes candidates at all experience levels.
Key Responsibilities:
- Conduct thorough assessments of patients with cognitive and physical functioning challenges, developing tailored care plans that adhere to clinical standards.
- Implement evidence-based practices and standardized assessments to enhance clinical interventions.
- Deliver rehabilitative services that are medically necessary, ensuring compliance with regulatory requirements.
- Administer a variety of therapeutic procedures as part of individualized rehabilitation plans.
- Monitor patient progress and adjust treatment strategies as needed, documenting all changes in clinical records.
- Evaluate treatment outcomes to ensure effectiveness, modifying services based on patient evaluations.
- Provide guidance and support to patients, families, and other healthcare providers regarding physical health issues.
- Maintain effective communication with supervisors and interdisciplinary team members regarding patient progress and treatment plans.
- Prepare comprehensive discharge summaries that meet regulatory standards.
