We are seeking a highly skilled Behavioral Health Care Advocate to join our team. As a Behavioral Health Care Advocate, you will play a critical role in supporting our members with complex behavioral health needs.
Key Responsibilities- Visit members in their homes and/or other settings to provide support and guidance
- Develop and implement personalized care plans to address members' unique needs
- Collaborate with healthcare providers and community resources to ensure seamless care coordination
- Provide education and support to members on managing their behavioral health
- Licensed Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling
- Active, unrestricted, independent license in Virginia: LCSW, LMFT, LPC, or LP
- 1+ years of experience working in behavioral health
- Proficient computer skills and good working knowledge of Microsoft Word and Outlook
- 1+ years of experience working with Medicaid population
- 1+ years of Community Mental Health experience
- 1+ years of experience working with local and state community resources in Virginia
