About the Role

The Department of Developmental Services (DDS) is seeking a skilled Licensed Practical Nurse II to join our team as a Community Care Specialist. As a key member of our healthcare team, you will be responsible for providing exceptional nursing care and oversight to individuals with developmental disabilities in community-based settings.

Key Responsibilities
  • Healthcare Services: Implement individualized healthcare plans, chart vital signs, apply medical dressings, and administer medications as prescribed by physicians.
  • Personal Care and Support: Assist with daily living activities, personal care, and other support services outlined in individual service plans.
  • Referrals and Coordination: Initiate referrals for medical services, schedule appointments, and engage in advocacy and follow-up care.
  • Communication and Documentation: Maintain accurate records, communicate health issues to staff, and update families/guardians on hospitalizations and other medical matters.
  • Medication Management: Order medications, maintain medical supplies, and ensure proper use of equipment and treatments.
  • Emergency Response: Apply basic first aid techniques, CPR, and tracheotomy care as needed.
  • Training and Education: Provide training to direct support staff on equipment use and treatments.
Requirements
  • Licensure: Possess a current, active license to practice as an LPN in the Commonwealth of Massachusetts.
  • Motor Vehicle Operator's License: Hold a valid Class D Motor Vehicle Operator's License.
  • Healthcare Provider Basic Life Support (BLS) Certification: Obtain and maintain BLS certification.

  • Relationship Building: Develop and maintain respectful, caring, and empowering relationships with individuals with intellectual and developmental disabilities.
  • Physical Abilities: Assist individuals with physical disabilities and/or limited mobility.
  • Nursing Knowledge: Possess knowledge of standard nursing policies, procedures, methods, and techniques.
  • Record Keeping: Maintain accurate records and exercise discretion in handling confidential information.
  • Communication Skills: Transcribe, read, and comprehend the English language.
