Job Summary

The Clinical Pharmacist position at the Veterans Affairs, Veterans Health Administration is a critical role in providing high-quality patient care and medication management services. As a key member of the interdisciplinary healthcare team, the Clinical Pharmacist will be responsible for ensuring the safe and effective use of medications, providing patient education and counseling, and participating in quality improvement initiatives.

Responsibilities
  • Conduct medication therapy reviews and provide recommendations to healthcare providers to optimize patient care.
  • Develop and implement medication use policies and procedures to ensure safe and effective medication use.
  • Provide patient education and counseling on medication use, side effects, and potential interactions.
  • Participate in quality improvement initiatives to improve medication safety and patient outcomes.
  • Collaborate with healthcare providers to develop and implement individualized patient care plans.
  • Conduct medication reconciliation and ensure accurate and complete medication lists.
  • Participate in interdisciplinary team meetings to discuss patient care and medication management.
  • Stay current with new medications, therapies, and treatment options to provide evidence-based care.
Requirements
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
  • Ability to communicate effectively with patients, healthcare providers, and other team members.
  • Strong analytical and problem-solving skills to identify and resolve medication-related issues.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
Preferred Qualifications
  • Successful completion of a PGY-1 pharmacy residency program.
  • Experience in medication therapy management, patient education, and quality improvement initiatives.
