Job Description:

Baptist Health is seeking a highly skilled Clinical Pharmacist to join our team at 1700 Nicholasville Road, Suite 1100, Lexington, KY. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and services.

Key Responsibilities:
  • Provide primary point of contact for patients related to specialty medication needs.
  • Enroll patients into medication management programs by assisting with recruitment and patient education.
  • Provide initial clinical medication counseling and regular patient reassessments to ensure appropriateness of specialty therapy and ongoing therapy management.
  • Perform medication therapy management and communicate with physicians and other clinicians as necessary.
  • Provide limited/indirect supervision for Specialty Pharmacy Care Coordinators and participate in ongoing trainings related to role.
Requirements:
  • Bachelor's degree in Pharmacy or PharmD required.
  • Kentucky Board of Pharmacy license required.
  • Experience in ambulatory clinical pharmacy practice preferred.
  • Specialty disease state experience preferred.
  • 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
