We are seeking a skilled Licensed Practical Nurse to join our team at Community Rehabilitation Hospital North. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to our hospital's mission to maximize the health, function, and quality of life of those we serve.
About the RoleThis is a full-time position working day shifts in our inpatient acute rehabilitation hospital. You will work closely with our interdisciplinary team to provide specialized care to patients recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.
Responsibilities- Assess and reassess patients to identify their individual needs and develop a plan of care that addresses their unique requirements.
- Administer medications and treatments as prescribed by physicians and other healthcare professionals.
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
- Provide education and support to patients and their families to promote optimal health outcomes.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Graduate of an accredited Licensed Practical/Vocational Nurse program.
- Current state licensure as a Licensed Practical/Vocational Nurse.
- CPR/BCLS certification. ACLS preferred.
- IV Certified required or obtained within 6 months highly preferred.
- Minimum six months' Medical/Surgical experience in an acute care setting preferred.
