Job Summary:

Key Responsibilities:

Conduct thorough patient assessments to identify individual needs and develop personalized care plans
Provide routine care, including vital sign monitoring, medication administration, and wound care
Collaborate with healthcare professionals to ensure comprehensive care and coordinate services
Maintain accurate and detailed records of patient care and progress
Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination

Requirements:

Current LPN licensure in good standing
Experience working with individuals with intellectual, developmental, or cognitive disabilities
Excellent communication and interpersonal skills
Ability to work collaboratively as part of a multidisciplinary team
