Lakeland Care is seeking a highly skilled and compassionate Care Manager Float to join our team. As a Care Manager Float, you will play a vital role in providing comprehensive social service support to our members in need.
Key Responsibilities- Conduct thorough assessments of members' outcomes, needs, and risks to develop effective care plans.
- Coordinate care and benefits to ensure a comprehensive support approach, including access to federal and state programs.
- Monitor and evaluate members' outcome-based plans, considering cost and effectiveness in authorizing services and choosing providers.
- Participate in home visits and care conferences to assess and reassess long-term care needs and coordinate appropriate interventions.
- Implement risk mitigation strategies to promote members' health, safety, and independence while respecting their rights.
- Maintain accurate and up-to-date member records as required by contract and policy.
- Participate in ongoing training to ensure compliance with Federal and State regulations, Lakeland Care policy, and accepted professional standards.
- Certified Social Worker in the State of Wisconsin with a minimum of one year of experience working with Family Care target populations, or
- A four-year bachelor's degree or more advanced degree in Human Services or a related field with one year of experience working with Family Care populations, or
- A four-year bachelor's degree or more advanced degree in any other area with a minimum of three years of experience working with Family Care target populations.
- Ability to be adaptable, prioritize, and work in a fast-paced environment.
- Working knowledge of computers, computer programs, typing, and data entry.
- Ability to access members' homes, which must comply with ADA regulations.
- Ability to lift up to 25lbs.
- Current driver's license, acceptable driving record, and proof of adequate insurance.
