Patient Care Associate

We are seeking a skilled Patient Care Associate to join our team at Hackensack Meridian Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.
Responsibilities- Provide personalized hygiene and comfort care to patients, including bathing, toileting, feeding, and bed making.
- Take vital signs under the direction of the RN and document electronically according to Medical Center policy.
- Perform and document glucose finger stick/heel stick and collect other specimens as directed by the RN.
- Monitor patients' intake and output and maintain accurate records.
- Assist with patient transport and provide a safe patient environment by responding promptly to call lights and communicating with the RN for any changes in patient condition.
- Act as a patient sitter when needed and maintain a clean and organized patient environment.
- Identify the needs of the patient population served and modify and deliver care that is specific to those needs.
- Communicate effectively with patients and primary caregivers at their level of understanding.
- High School Diploma or Equivalent
- Completion of a Certified Nursing Assistant, Medical Assistant, or Patient Care Technician program or equivalent medical background experience.
- The ability to learn quickly and adapt to changing patient needs, a strong sense of accountability for improving the lives of our patients and their caregivers, and an exceptional focus on teamwork.
