Job Description

This position involves driving to patients' homes and conducting home health visits independently. The ideal candidate will have a strong background in case management and be comfortable working with patients who have experienced challenging situations.

Responsibilities:
  • Driving to patients' homes and conducting home health visits independently
  • Obtaining consent, checking patient history, and administering immunizations
Qualifications:

To be successful in this role, you must have a valid LPN license, a NY driver's license, and an acceptable driving record. Being bilingual in Spanish is preferred, but not required. You should also be comfortable driving and doing home health visits independently.

Experience:

Highly desired experience includes social services, emergency type care, detention centers, and homeless shelters.

Compensation:

The estimated salary for this role is between $40.00 and $42.00 per hour, making it a competitive opportunity for LPNs looking to make a difference in the lives of others.
