Job Title: Caregiver Healthcare Specialist
About the Position:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Services for the UnderServed. As a Caregiver Healthcare Specialist, you will play a vital role in providing high-quality patient care to individuals residing at our adult family shelter in Manhattan.
About the Role:
The LPN will be responsible for coordinating with healthcare providers to ensure residents receive ongoing treatment in the community. Key responsibilities include taking vital signs, assisting with diabetes medication and treatment, and documenting medical histories in computer systems. You will also work closely with case managers, senior case managers, and directors of social services to ensure proper care, nutrition, and treatment plans are followed.
Requirements:
To be successful in this role, you must have completion of an accredited program and licensure as an LPN in New York. Preferred qualifications include bilingual skills and the ability to work independently and collaboratively as part of a team. Salary Information:
The estimated salary range for this position is $55,000 to $60,000 per year, based on industry standards and location.
