We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Mass General Brigham. As a key member of our team, you will provide high-quality patient care and support to individuals in their homes, promoting independence and well-being.
Responsibilities- Provide skilled nursing care to patients in their homes, assessing their needs and developing individualized care plans.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and their families.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure effective care delivery.
- Provide education and support to patients and their families, empowering them to manage their health and well-being.
- Document patient progress and outcomes, maintaining accurate and detailed records of care provided.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Current CPR certification required.
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
- Demonstrated organizational and time management skills required.
- Current driver's license, a good driving record, and reliable transportation required.
