Job Description:

The primary responsibility of the Admissions Nurse includes completing the admission process, which entails assessing, planning, implementing, and evaluating patient care plans. You will welcome patients and assist with the move-in process, deliver the Welcome Packet, answer questions, and begin the C.A.R.E.S. process. Furthermore, you will coordinate communication with family members, Nursing Home Staff, Social Workers, and other teams regarding the admission process.

Key responsibilities include:

  • Completing initial plans of care in the Electronic Medical Record (EMR)
  • Conducting head-to-toe assessments and verifying that all necessary treatments and orders are in place
  • Initiating preventative measures, such as skin, fall risk, elopement, etc.
  • Completing admission paperwork, including initial plans of care, medication lists, admit papers, physician orders, code status papers, and other required documentation
Requirements and Qualifications:

To excel in this role, you must possess the following qualifications:

  • Graduate of an accredited school of nursing
  • Bachelor's or Master's degree preferred
  • Current nursing license in good standing in the state of employment
  • Minimum two years of varied work experience as a professional nurse, preferably one year in a post-acute setting
  • Strong administrative performance skills, including expertise in written and oral communications and documentation requirements
  • Able to assess and respond to the needs of patients and families in various settings
  • Demonstrated skill in nursing practice
  • Ability to cope with family emotional stress and tolerance of individual lifestyles
  • Ability to lift, transfer, and/or move items or patients weighing 10-200+ pounds using proper lifting techniques and requesting assistance when necessary
