About the RoleThe Nurse Practitioner will be part of a well-established group, working collaboratively with a team of healthcare professionals to deliver exceptional patient care. You will be responsible for providing comprehensive primary care services, including health assessments, diagnosis, and treatment of acute and chronic conditions.
Responsibilities- Provide high-quality patient care to the adolescent, adult, and geriatric populations
- Conduct health assessments, diagnose, and treat acute and chronic conditions
- Develop and implement individualized care plans to meet the unique needs of patients
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care
- Graduated from an accredited Nurse Practitioner program
- Active License in the State of New Jersey
- ACLS and BLS certification required
- Minimum of 2 years of primary care experience required
- 5 years of experience preferred
