LPN Care Specialist

Job Summary:
* The LPN working in the Transitional Care Unit is responsible for providing direct care to adult/geriatric patients experiencing prolonged general medical or post-operative recovery.
Key Responsibilities:
* Provide medication administration, dressing changes, monitoring of vital signs and skin integrity
* Document patient information in the medical record
* Support and educate patients and families on their care plan
Requirements:
* Graduate of an accredited practical nursing program
* Previous nursing experience preferred
* Excellent verbal and written communication skills
* Ability to work in a fast-paced environment with flexibility and adaptability
