**Responsibilities:**

* Assist clients with personal care, including bathing, dressing, and grooming
* Provide medication reminders and assist with medication management
* Prepare meals and snacks according to client's dietary needs
* Assist with therapy exercises and ambulation
* Maintain a safe and clean environment, including light housekeeping and laundry
* Provide companionship and stimulation, including reading, walks, and accompanying clients to appointments
* Monitor client's condition and report any changes to the Agency Director or Service Coordinator

**Requirements:**

* High school diploma or equivalent required
* One year of personal care experience or completion of a Home Health Aide training program
* Ability to read, write, and speak English
* Reliable transportation and ability to travel to client's homes
* Current CPR, First Aid, and TB test certifications
