Job Summary

Sunbelt Staffing is seeking a skilled Hospice and Home Health Registered Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and coordination in a home health and hospice setting.

Responsibilities
  • Conduct thorough patient assessments and develop individualized care plans to meet the unique needs of each patient.
  • Manage a caseload of patients, ensuring timely and effective care coordination with other healthcare professionals.
  • Collaborate with physicians, caregivers, and insurance companies to ensure seamless care delivery.
Requirements
  • Minimum one year of experience in home health and hospice care within the last year.
  • Proficient knowledge and field experience of OASIS (Outcome and Assessment Information Set) required.
  • Start of Care/Admissions experience required.
  • Experience in case management, handling a high volume of patients per day and week.
Education and Certifications
  • Associate's Degree of Nursing or Bachelors Degree of Nursing.
  • Ohio Registered Nurse License or compact license.
  • American Heart Association Basic Life Support certification.
Work Environment

8:00 AM - 5:00 PM, 40 hours per week, with some on-call and weekend requirements.
