About the Role

We're looking for a compassionate and experienced Registered Nurse to provide skilled nursing visits, case management, and follow-up care to our patients. As a Home Health Registered Nurse, you will:

  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and effective communication.
  • Provide hands-on care, education, and support to patients and their families, promoting independence and optimal health outcomes.
  • Document patient progress, update care plans, and communicate with physicians and other healthcare professionals as needed.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements

To be successful in this role, you will need:

  • A diploma, associate, or bachelor's degree in nursing.
  • At least one year of nursing experience, preferably in a home health or acute care setting.
  • Strong clinical skills, excellent communication, and interpersonal skills.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
