CenterWell Home Health is seeking a dedicated and compassionate Registered Nurse to join our team. As a Home Health Nurse, you will provide personalized care to patients in the comfort of their own homes, helping them to recover from illness, injury, or surgery.

About the Role

As a Home Health Nurse, you will be responsible for:

  • Providing skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care.
  • Conducting thorough assessments of patients' physical and emotional needs, and developing personalized care plans to meet those needs.
  • Collaborating with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Documenting patient progress and outcomes, and making adjustments to care plans as needed.
  • Providing education and support to patients and families on disease management, medication adherence, and healthy lifestyle choices.
Requirements

To be successful in this role, you will need:

  • A diploma, associate, or bachelor's degree in nursing.
  • At least one year of experience in nursing, preferably in home health or a related field.
  • Current and unrestricted RN licensure.
  • Current CPR certification.
  • Strong organizational and communication skills, with the ability to work independently and as part of a team.
