Job Summary

Park Avenue Healthcare & Wellness Centre is seeking a highly skilled and experienced Licensed Vocational Nurse (LVN) to fill the role of Charge Nurse. As a key member of our nursing team, the successful candidate will be responsible for coordinating daily administrative duties, supervising nursing staff, and ensuring the delivery of high-quality patient care.

Key Responsibilities
  • Administrative Duties: Coordinate daily schedules, nursing assignments, and delegation of resident care.
  • Supervision: Supervise and support nursing staff, monitor compliance with facility policies and procedures, and ensure adherence to state and federal regulations.
  • Clinical Responsibilities: Maintain overall shift operation, coordinate resident admissions, transfers, and discharges, and ensure the implementation of individualized care plans.
  • Communication: Communicate effectively with patients, families, medical staff, and other departments to ensure seamless care delivery.
  • Quality Assurance: Participate in the facility's Quality Assurance program, identify areas for improvement, and contribute to the development of corrective actions.
Requirements
  • Graduate of an accredited school of nursing or an approved LVN program.
  • Current nursing license in good standing with the State licensing board.
  • Experience in a long-term care setting, preferred.
  • Strong leadership, communication, and organizational skills.
  • Ability to provide patient care in accordance with applicable standards, policies, and procedures.
