We are seeking a skilled Per Diem Community Care Nurse Practitioner or Physician Assistant to join our team at Optum. This is an exciting opportunity for professionals looking to make a meaningful difference in the lives of patients.
Job SummaryThis role involves providing cost-effective, quality care to assigned members. You will be responsible for delivering comprehensive assessments and documenting findings that comply with documentation requirements and regulations. Your expertise will also involve ensuring accurate diagnoses, coding, and medication reviews. You will work collaboratively with physicians, specialty providers, facility staff, families, and primary care physicians to facilitate agreement and implementation of member plans of care.
Care Coordination- Understand Payer/Plan benefits, CCM associate policies, procedures, and articulate them effectively to providers, members, and key decision-makers.
- Assess medical necessity/effectiveness of ancillary services to determine initiation of benefit events and communicate process to providers and team members.
- Coordinate care as members transition through different levels of care and care settings.
- Certified Nurse Practitioner through a national board.
- Active and unrestricted license in the state where you reside.
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).
- Access to reliable transportation for travel to client and/or patient sites within a designated area.
- 1+ years of hands-on post-grad experience within Long Term Care.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient computer skills including documentation of medical information with written and electronic medical records.
The hourly range for this role is $42.31 to $83.27.
