Senior Care Nurse Practitioner Opportunity
At Abode Care Partners, we are seeking a skilled and compassionate Senior Care Nurse Practitioner to provide high-quality medical care to seniors and individuals with complex needs. As a key member of our team, you will be responsible for delivering evidence-based medical care, examining and treating chronic and urgent illnesses, ordering diagnostic testing, and collaborating with healthcare professionals to develop comprehensive patient care plans.
Key Responsibilities:
- Perform medical assessments and develop personalized care plans
- Order diagnostic testing and interpret results
- Collaborate with healthcare professionals to ensure seamless care
- Provide education and support to patients and families
Requirements:
- Active NP license
- Current national board certification
- At least five years of nursing experience
