**Job Summary**

**Responsibilities:**

  • Provide comprehensive primary care services to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with our multidisciplinary team to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Stay up-to-date with the latest clinical guidelines and best practices in family medicine to provide evidence-based care.
  • Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.

**Requirements:**

  • Current licensure as a Nurse Practitioner in the state of Arizona.
  • Board certification in Family Medicine or a related specialty.
  • Minimum of 2 years of experience as a Nurse Practitioner in a primary care setting.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
