Position Overview:
We are seeking a Hospice Nurse Practitioner to fulfill a vital role within our organization. Reporting directly to the Executive Director or Administrator, you will be assigned to conduct Face-to-Face (F2F) assessments and gather essential clinical data to substantiate hospice diagnoses and prognoses. Your findings will be communicated to the hospice physician responsible for certifying or recertifying the patient's eligibility for hospice services.

The responsibilities of the hospice NP are specifically focused on tasks required to complete hospice F2F encounter visits.

  • Conduct and finalize physical assessments for the F2F encounter within the regulatory time frames established by CMS.
  • Provide clinical findings, including evaluations of medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility.
  • Make appropriate recommendations to the Interdisciplinary Group (IDG) based on the findings from the F2F assessment.
  • Notify the hospice agency if unable to complete a F2F visit as scheduled.
Qualifications:
  • Master's degree or higher in Nursing.
  • Completion of an accredited graduate-level nurse practitioner education program.
  • Current state license as a registered nurse.
  • Current state license or certification as an advanced practice nurse.
  • Current national certification as a Nurse Practitioner (ANCC or AANP).
  • A collaborative agreement must be in place.
