ROLE OVERVIEW
The Jewish Community Support Specialist will advocate for client rights and facilitate access to case management services tailored specifically for the Jewish community in Western New York. This role involves working in a culturally sensitive and client-focused manner with Jewish individuals and families, irrespective of their Temple affiliation, and may extend services to non-Jewish partners of community members to assist with healthcare and social needs.
Key Responsibilities
Client Support
- Guide community members and their families through the healthcare and social services landscape, including home visits, financial management, and access to public benefits and medical appointments.
- Provide assistance to community members and their families in navigating the healthcare and social services system, ensuring comprehensive support.
Case Management
- Develop treatment plans, discharge strategies, and follow-up care programs; deliver therapeutic crisis intervention and emergency services as necessary, followed by referrals to suitable resources.
- Gather client data through interviews, case histories, psychological assessments, and observational methods; analyze data to identify issues and determine appropriate therapeutic approaches or referrals.
Referrals
- Collaborate with healthcare providers, social service organizations, and other patient services to enhance patient care and outcomes while ensuring cultural and linguistic accessibility.
- Facilitate client registration for appointments with relevant staff members.
Administrative Duties
- Utilize interpretation services to guarantee clients receive assistance without language barriers.
- Complete intake forms and needs assessments efficiently.
- Process new client documentation into the electronic medical record promptly and in compliance with established protocols.
- Maintain and organize case files as required.
- Perform clerical tasks such as data entry, mailing, filing, copying, and updating records.
- Handle sensitive agency-related information confidentially, adhering to HIPAA and other regulatory guidelines.
Advocacy Efforts
- Engage with other therapists and relevant professionals in therapeutic and casework processes; refer clients to appropriate service agencies as needed.
QUALIFICATIONS
- A minimum of a Bachelor's degree in Social Work (MSW preferred), human services, or a related field is required.
- Experience in case management is essential.
- Familiarity with local community resources and support for Holocaust survivors is preferred.
- Proven ability to maintain accurate case notes and documentation.
- Flexibility to work in both office and community settings, including evenings and weekends for community events.
- Strong organizational and time management skills; ability to prioritize tasks effectively.
- Proficient in computer and internet applications, particularly Microsoft Office and Outlook.
- Excellent written and verbal communication skills.
- Commitment to maintaining confidentiality at all times.
- Exceptional client-centered service orientation.
This position requires travel between various sites, necessitating a reliable vehicle, valid driver's license, and appropriate liability insurance. Additionally, proof of Covid-19 vaccination is required upon hiring.
BENEFITS
- Competitive hourly wage ranging from $21.00 to $28.00, based on experience and qualifications.
