Under the supervision of a Registered Nurse, the Licensed Practical Nurse coordinates the Plan of Care for patients in the home, hospital, or other settings. This role involves assessing, planning, and implementing care plans, working cooperatively with other team members to maintain standards of professional nursing practice.
Key Responsibilities- Assess patient needs and develop individualized care plans
- Implement care plans in collaboration with other healthcare professionals
- Provide education and support to patients and their families
- Document patient progress and outcomes
- Current licensure to practice as a Licensed Practical Nurse in NYS
- Minimum one (1) year experience in Acute Medical/Surgical Nursing
- Valid drivers license
This position is based in Home Care-Erie County and requires a valid drivers license for travel to patient locations.
