Job Description:

We are seeking a compassionate Nurse Practitioner to join our team. As a Geriatric Medical Care Specialist, you will provide evidence-based medical care to seniors and patients with complex needs. Your responsibilities will include evaluating and treating chronic and urgent illnesses, collaborating with other healthcare professionals, and ensuring comprehensive patient care.

Responsibilities:

  • ">
  • Perform evidence-based medical care by planning, directing, and evaluating patient medical needs">
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses">
  • Order and interpret laboratory and diagnostic testing as needed">
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care">
  • Provide referrals to other healthcare practitioners when necessary">
  • Ensure patient medical records are kept up-to-date">

Required Skills and Qualifications:

">

To be successful in this role, you must have an active state Nurse Practitioner license, current Nurse Practitioner National Board Certification, and active state DEA licensure. A Master's degree in Nursing (MSN) and five or more years of nursing experience, with at least two years of nurse practitioner experience, are also required. You should possess strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment.
