Position Overview:

The Practical Nursing Specialist plays a crucial role in supporting the management and enhancement of self-care strategies for patients within primary care settings.

This role emphasizes exceptional teamwork and communication with the Core Team, which includes the Patient, Primary Care Provider, RN Care Manager, and Clerk, as well as the Expanded Team comprising family members and community-based services. The aim is to ensure timely access to comprehensive services and seamless coordination to elevate the quality of patient care.

Eligibility Requirements:

Citizenship:

Applicants must be U.S. citizens. Non-citizens may only be considered when qualified citizens are unavailable, in accordance with relevant policies.

Educational Background:

Candidates must be graduates of a practical or vocational nursing program accredited by the appropriate State agency or the National League for Nursing Accrediting Commission (NLNAC).

Licensure:

Possession of a full, active, current, and unrestricted license as a practical or vocational nurse in a U.S. State, Territory, or Commonwealth, or the District of Columbia is required.

Individuals with a history of license revocation, suspension, or probation may be appointed only under specific provisions.

Physical and Language Proficiency:

Applicants must meet physical standards as outlined in relevant directives and demonstrate proficiency in both spoken and written English.

Grade Determinations:

Applicants may be considered for various grade levels based on their experience and qualifications:

  • GS-3: No additional requirements beyond basic qualifications.
  • GS-4: Requires six months of qualifying experience or relevant education.
  • GS-5: Requires one year of experience at the GS-4 level or equivalent.
  • GS-6: Requires one year of experience at the GS-5 level or equivalent, demonstrating proficiency in patient care.

Key Responsibilities:

The Practical Nursing Specialist is expected to:

  • Utilize veteran care data to contribute actively to the overall health plan of patients.
  • Monitor and document changes in patients' conditions, implementing interventions to enhance their health outcomes.
  • Collaborate with Registered Nurses and Health Care Providers to develop and modify treatment plans.
  • Administer prescribed medications and perform treatments according to established protocols.
  • Respond effectively to urgent situations, ensuring timely communication with appropriate personnel.

This position requires a commitment to fostering a respectful environment that prioritizes patient dignity and privacy in all aspects of care delivery.

Work Schedule: Standard working hours with rotating weekends.
