Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at Mercy. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to meet their unique requirements.
  • Provide skilled nursing services, including wound care, medication management, and vital sign monitoring.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document patient progress and communicate effectively with the healthcare team.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current LPN license in the state of Missouri.
  • One year of experience in a related field, preferably in home health or hospice care.
  • BLS CPR Certification through American Heart Association.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Flexibility to work a part-time schedule, with varying shifts and on-call responsibilities.
