Care Manager Specialist, Full-Time Position

We are seeking a skilled Care Manager Specialist to join our team in a full-time capacity. This role requires a unique blend of social work skills and healthcare knowledge to provide comprehensive care coordination services to individuals and families facing complex challenges.
About the RoleThe Care Manager Specialist will be responsible for conducting thorough assessments of clients' social, emotional, and environmental circumstances, developing personalized care plans, and coordinating access to necessary services and resources. This position will also engage in crisis intervention, counseling, and advocacy, while promoting preventive measures and education within the community.
Responsibilities- Conduct thorough assessments of clients' social, emotional, and environmental circumstances.
- Develop and implement personalized care plans tailored to each client's needs.
- Cordinate access to necessary services and resources, including healthcare providers, community organizations, and government agencies.
- Engage in crisis intervention, counseling, and advocacy to support clients in achieving their goals for health and social stability.
To be successful in this role, you must possess a Master's degree in Social Work and be licensed as a Social Worker in the state where you reside. A minimum of 2 years' experience in an acute care hospital setting is preferred. Certification in Case Management is also desirable.
What We OfferWe offer a competitive salary range of $63,000 - $85,000 per year, based on experience, as well as a comprehensive benefits package, including health, vision, and dental coverage, paid time off, and retirement savings plan.
