Senior Patient Care Coordinator

We are seeking a highly skilled and experienced Senior Patient Care Coordinator to join our team at UnitedHealth Group. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and administrative support to our patients and healthcare providers.
Key Responsibilities- Verify patient insurance coverage and eligibility, and update demographic and insurance information as needed.
- Collect and post patient co-payments and other outstanding balances.
- Generate and track patient encounter forms, and review them for completeness and accuracy.
- Provide routine information on procedures and standard policies to patients, and refer matters to appropriate personnel as needed.
- Identify problem-related priorities and respond to emergency needs by contacting physicians or providers within guidelines.
- Resolve patient questions and concerns regarding clinic schedules and billing, and handle requests for prescription refills and rescheduled appointments.
- Document information in computer systems, prepare patient charts, and ensure completeness by locating test results and reminding patients of appointments.
- Update and maintain the Telephone Message System (TMS) in computer systems, and follow through on requests.
- Enter new referrals or ensure that existing referral numbers are linked in the system to meet managed care requirements.
- Provide patient education regarding managed care plans and referral processes, and answer patient questions and concerns.
- Work collaboratively with primary care practices, patients, and specialty practices to process outgoing referrals prior to scheduled visits, and follow up with practices that do not issue a referral or obtain a referral on a timely basis.
- Complete and maintain patient schedules, and schedule and coordinate patient visits and medical procedures for both inpatients and outpatients.
- Notify providers, patients, and others of changes such as new scheduling, rescheduling, no-shows, emergency appointments, and add-ons.
- Maintain recall lists and communicate with patients as needed.
- May escort patients to examination rooms and chaperone patients as required, and process standard and non-standard business office administrative paperwork.
- Provide guidance to departmental personnel on administrative policies and procedures.
- Comply with health and safety requirements and regulatory agencies such as DPH.
- Comply with established departmental policies, procedures, and objectives.
- Enhance professional growth and development through educational programs, seminars, and other opportunities.
- Attend meetings, conferences, and seminars as required or directed.
- Regular, reliable, and predictable attendance is required.
- Perform other similar and related duties as required or directed.
- High school diploma or equivalent required.
- 2+ years of hospital and/or patient registration and/or clinic experience required.
- Additional specialized medical secretarial/medical office training preferred.
- Superior customer service skills, including the ability to multi-task and resolve patient concerns in a timely manner.
- Excellent communication, interpersonal, and organizational skills.
- Familiarity with various software applications such as Microsoft Word, Outlook, Excel, and EPIC.
