Overview

The Physical Therapist I - PRN is responsible for evaluating and assessing patients to set functional, measurable objectives aimed at restoring their physical capabilities. Treatment is provided in accordance with a documented care plan to achieve the defined goals.

Discharge planning is an essential part of the role, along with communicating findings and progress in line with professional licensure, scope of practice, and departmental policies.

Additional reports may be generated as required. The position may involve supervising clinical students and support personnel, including certified assistants and volunteers. A compassionate relationship is fostered to enhance health and healing. Other responsibilities may be assigned as necessary.

Key Responsibilities

1. Conduct thorough evaluations and assessments of patients using both subjective and objective data.

2. Formulate a comprehensive written care plan.

3. Deliver treatment in alignment with the established goals outlined in the care plan.

4. Accurately document all interventions and care provided in the patient's medical records.

5. Assess the effectiveness of the care plan and continuously evaluate the patient's functional status.

6. Provide essential discharge planning information, including education for patients and families, follow-up care, and referrals to community resources.

7. Contribute to the professional development of staff and students.

8. Engage in initiatives aimed at enhancing healthcare delivery.

9. Complete assigned objectives in a timely manner.

Qualifications

Experience:
Entry-level position.

Licensure, Registration, Certification:
BLS Certification required within 14 days of hire.

Special Skills:
Basic proficiency in keyboarding, mouse usage, and navigating digital platforms.
