Position Overview

As a Physical Therapist I, you will be responsible for evaluating and assessing patients to establish functional and measurable objectives aimed at restoring their physical capabilities. You will deliver treatment based on a structured plan of care designed to achieve these objectives.

Additionally, you will prepare for patient discharge and effectively communicate your findings and progress in accordance with professional standards, licensure, and departmental policies.

You may also be involved in supervising clinical students and support personnel, including certified assistants and technicians, while fostering a supportive environment that promotes health and recovery. Other responsibilities may be assigned as needed.

Key Responsibilities

1. Conduct thorough evaluations and assessments of patients utilizing both subjective and objective data.

2. Develop a comprehensive written plan of care tailored to each patient's needs.

3. Administer treatment in alignment with the established goals outlined in the plan of care.

4. Maintain accurate documentation of interventions and care provided in the patient's medical records.

5. Assess the effectiveness of the care plan and make necessary adjustments based on the patient's functional status.

6. Provide discharge planning information, including education for patients and families, follow-up care, and referrals to community resources.

7. Contribute to the professional development of staff and students.

8. Engage in initiatives aimed at enhancing healthcare delivery.

9. Fulfill assigned performance goals.

Qualifications and Experience

Minimum Experience:

Licensure and Certification:
Minimum:
BLS Certification required within 14 days of hire.

Proficient in basic computer skills, including keyboarding and navigating online platforms.
