Role Responsibilities
  • Manage and communicate patient referrals and authorizations to healthcare providers.
  • Coordinate and relay appointment details to patients and their families.
  • Conduct follow-ups with patients and providers to confirm appointment attendance.
  • Document consults and progress notes from healthcare professionals in patient records.
  • Administer pre-intake assessments for Medication Assisted Treatment (MAT) to evaluate program suitability.
  • Record MAT group attendance, perform random drug screenings, and maintain inventory counts.
  • Complete assessments related to Social Determinants of Health (SDoH) and track outcomes in patient records.
  • Ensure adherence to Telepsychiatry protocols and coordinate care with remote providers.
  • Maintain accurate documentation of all actions in compliance with clinic and health plan guidelines.
  • Discuss referral processes with patients, including financial considerations when necessary.
  • Engage with patients through various communication methods.
  • Act as a liaison among patients, healthcare providers, insurance plans, and community resources.
  • Participate in required meetings and training sessions.
  • Verify insurance eligibility and assist with treatment authorizations.
  • Contribute to quality improvement initiatives.
  • Document care coordination and patient education efforts in medical records.
  • Initiate incident reports following established documentation protocols.
  • Prioritize tasks based on urgency and required follow-up.
  • Provide timely reports to supervisors and site staff as needed.
  • Respond compassionately to the needs of patients and colleagues.
  • Foster professional relationships with staff, clients, and the community.
  • Collaborate to achieve departmental goals and objectives.
Qualifications

Education and Experience

  • High school diploma or GED is mandatory.
  • Completion of a Medical Assistant program from an accredited institution is required.
  • Preferred: One year of experience as a certified medical assistant.
  • Current BLS certification is required upon hire.
  • Bilingual proficiency in English and Spanish is preferred.

Skills and Abilities

  • Understanding of medical terminology and procedures.
  • Experience with electronic health records is preferred.
  • Intermediate skills in computer usage and data entry.
  • Ability to anticipate the needs of patients and providers with sound judgment.
  • Adaptability to changing priorities and time-sensitive tasks.
  • Utilize emotional and cultural intelligence in patient interactions.

Compensation: $32.55 per hour, commensurate with experience.
