Role Responsibilities- Manage and communicate patient referrals and authorizations to healthcare providers.
- Coordinate and relay appointment details to patients and their families.
- Conduct follow-ups with patients and providers to confirm appointment attendance.
- Document consults and progress notes from healthcare professionals in patient records.
- Administer pre-intake assessments for Medication Assisted Treatment (MAT) to evaluate program suitability.
- Record MAT group attendance, perform random drug screenings, and maintain inventory counts.
- Complete assessments related to Social Determinants of Health (SDoH) and track outcomes in patient records.
- Ensure adherence to Telepsychiatry protocols and coordinate care with remote providers.
- Maintain accurate documentation of all actions in compliance with clinic and health plan guidelines.
- Discuss referral processes with patients, including financial considerations when necessary.
- Engage with patients through various communication methods.
- Act as a liaison among patients, healthcare providers, insurance plans, and community resources.
- Participate in required meetings and training sessions.
- Verify insurance eligibility and assist with treatment authorizations.
- Contribute to quality improvement initiatives.
- Document care coordination and patient education efforts in medical records.
- Initiate incident reports following established documentation protocols.
- Prioritize tasks based on urgency and required follow-up.
- Provide timely reports to supervisors and site staff as needed.
- Respond compassionately to the needs of patients and colleagues.
- Foster professional relationships with staff, clients, and the community.
- Collaborate to achieve departmental goals and objectives.
Education and Experience
- High school diploma or GED is mandatory.
- Completion of a Medical Assistant program from an accredited institution is required.
- Preferred: One year of experience as a certified medical assistant.
- Current BLS certification is required upon hire.
- Bilingual proficiency in English and Spanish is preferred.
Skills and Abilities
- Understanding of medical terminology and procedures.
- Experience with electronic health records is preferred.

Compensation: $32.55 per hour, commensurate with experience.
