Job Title: Physical Therapist (PT) SNF / LTC

Centra Healthcare Allied is seeking a skilled Physical Therapist to join their team for a contract assignment in Suffolk, Virginia.

Job Summary:

The Physical Therapist will be responsible for assessing patients' medical conditions, functional capabilities, limitations, and restrictions, and developing a treatment plan to improve their physical function and mobility.

Responsibilities:
  • Assess patients' medical conditions and develop a treatment plan to improve their physical function and mobility.
  • Provide direct patient care, including physical therapy interventions and education.
  • Collaborate with other healthcare professionals to develop and implement a comprehensive treatment plan.
  • Document and record patients' progress and treatment plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care.
Requirements:
  • Graduate of an accredited Physical Therapy program.
  • Current state license as a Registered Physical Therapist or in the process of obtaining licensure.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
