RN Care Navigator for Heart Transplant Patients

Job Summary
UC Health seeks an experienced RN to fill the position of Ambulatory RN Care Coordinator for our Heart Transplant program. This is a unique opportunity to work with patients who require specialized care and coordination.
About the Role
In this role, you'll be responsible for ensuring seamless transitions between outpatient and inpatient settings. You'll develop and implement individualized care plans, collaborate with interdisciplinary teams, and provide education to patients and families.
Responsibilities
The successful candidate will have a strong background in nursing and experience working with heart transplant patients. Key responsibilities include:
- Supporting ambulatory physician clinics by seeing patients with complex and chronic disease
- Working with patients to develop and execute care plans
- Providing patient education and support
- Maintaining awareness of comfort and safety needs of patients during their ambulatory appointment
Requirements
To be considered for this role, you must have a current Registered Nurse (RN) license in the state of Ohio and a minimum of 2 years of experience in an ambulatory or outpatient setting. A Bachelor's Degree in Nursing is preferred.
What We Offer
We offer a competitive salary range of $90,000 - $120,000 per year, depending on experience. Additional benefits include comprehensive health insurance, paid time off, and professional development opportunities.
