Healthcare Professional | Advanced Practice Clinician

We are seeking a skilled Advanced Practice Clinician to join our Optum Home and Community Care team. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities- Work as a provider member within the HBMC interdisciplinary team to deliver home-based medical care to patients in an assigned geography.
- Collaborate with PCPs, HBMC team members, and consultants to develop and implement patient care plans.
- Establish and maintain effective relationships with patients, caregivers, and healthcare professionals.
- Meet with patients to review care plans, complete follow-up assessments, and provide education on disease management and wellness.
- Implement HEDIS measure campaigns and quality initiatives to ensure high standards of care.
- Knowledge of long-term care, community resources, and cost-effective alternatives for patient care delivery systems.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
