Key Responsibilities:
As a Care Coordinator, you will be responsible for coordinating patient care needs, managing care delivery, discharge planning, and advocating for patients. You will work closely with the healthcare team to ensure individual patient needs are met across the care continuum.
Requirements:
- Bachelor's degree in Nursing (BSN) or enrolled in a BSN program
- Minimum 1 year of acute care experience
- Current RN license and BLS certification
- Skills in fast-paced, multitasking environment
- Computer proficiency
Work Environment:
- Regular full-time, night shift
