We are seeking a dedicated Nurse Practitioner to join our Hospice Care team. As a key member of our team, you will provide expert medical care to patients in their homes, focusing on symptom management and end-of-life care. Your primary responsibilities will include:

* Collaborating with our interdisciplinary team to develop comprehensive care plans tailored to each patient's needs and goals.
* Conducting face-to-face encounters with patients and documenting their progress and treatment plans.
* Working closely with healthcare professionals to ensure seamless care delivery and address any urgent medical concerns.

About the Ideal Candidate
To excel in this role, you must have a Master's degree from an accredited advanced nurse practitioner program, a current Oregon license, and at least one year of experience as a nurse practitioner. Certification in hospice and palliative care is a strong asset

This full-time position comes with a flexible schedule, including evenings and weekends, and requires travel outside the workplace at least weekly.
