Responsibilities:

  • Provide skilled nursing care to clients in their homes and in skilled care settings, including wound care, dressing changes, vent care, and IV medication administration.
  • Assist with daily living activities, such as bathing, dressing, grooming, and transferring, as well as providing companionship and running errands.
  • Collaborate with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care strategies.
  • Document client progress and communicate with the care team to ensure seamless care coordination.

Requirements:

  • Current state of Hawaii LPN or RN license
  • Current Adult CPR certification
  • Current First Aid certification
  • Current TB clearance
  • Criminal Abstract review
