Primary Duties:
  • Deliver therapeutic and case management support to adults dealing with substance use issues and mental health conditions.
  • Assist individuals in improving their community engagement through the implementation of collaborative care plans.
  • Ensure comprehensive and coordinated care for each client, including support with benefit and health insurance applications.
  • Facilitate access to vital resources such as psychiatric services, housing, rehabilitation, and social activities.
  • Conduct regular outreach to maintain client engagement in services.
  • Communicate consistently with other treatment providers to ensure holistic care.
  • Complete necessary documentation in the Electronic Health Record (EHR) system promptly.
  • Meet established billing targets on a weekly, monthly, and quarterly basis.
Required Qualifications:
  • Associate degree in psychology, social work, or a related discipline; Master's degree is preferred.
  • CDCA certification and a valid Ohio Driver's License.
  • A minimum of two years of experience working with adults facing substance use and/or mental health challenges.
  • Understanding of trauma and its implications.
  • Strong verbal and written communication abilities.
  • Excellent organizational skills with the capacity to manage multiple responsibilities.
  • Strong interpersonal skills to effectively engage with clients, colleagues, and the community.
  • Ability to adapt to clinical demands and emergency situations.
Physical Demands:
  • Regular sitting, use of hands and fingers, talking, and hearing.
  • Frequent walking, balancing, stooping, kneeling, and/or crouching.
  • Occasional lifting/moving of items up to 15 pounds.
  • Vision requirements include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Data entry via keyboard is necessary.
